Description: A palliative approach to care is the right of all persons who are known to have a life-limiting illness, but it has been found that those who are houseless or precariously housed are more likely to not have access to this support and care. The nuances to this approach to care are extensive for those in this person population, with collaboration of care teams being key to success. Cultural needs assessment and health centred engagement, along with skilled communication and education are essential, but not universal. This presentation will discuss the work being done as part of a regional program initiatives, with the support of an expert team and grant from Healthcare Excellence Canada.
Presenter: Janice Nesbitt RN MN CPHCN(C). Janice is the Clinical Operations Lead with the Winnipeg Regional Health Authority and serves as the clinical project lead for this initiative. She is a current member of the CPCNA Board and is passionate about this work and supporting the teams who are providing direct support to all persons supporting a palliative approach to care.
