Can we talk? Â How a shared language of care can promote a palliative approach to heart failure
How can a patient with advancing heart failure (HF) experience care that alleviates suffering and helps them to live their best life? While some progress has been made to integrate a palliative approach to the care of these patients and their families, challenges exist in bridging the two distinct care cultures of HF and Palliative Care. Essential to this important practice shift is the effective communication between health care providers, patients and their family caregivers. This presentation will explore how the Heart Failure Palliative Approach to Care (HeFPAC) was designed to build capacity for care through a shared language that fosters communication about priority issues that commonly arise in advanced HF.
